Your Role As a Licensed Medical Social Worker , you will be an essential advocate for our patients and their families providing emotional support, coordinating care, and ensuring that every individual receives the highest standard of service and compassion. What Youll Do Provide professional medical social work services in alignment with hospice standards of care. Conduct psychosocial assessments to understand each patients emotional and social needs. Collaborate closely with physicians, nurses, and families to create individualized care plans. Make follow-up visits to evaluate progress and adjust care strategies as needed. Develop interventions tailored to patients evolving needs and circumstances. Maintain accurate, timely, and confidential documentation.
