Nursing Supervisor This hybrid role requires a mix of case
management, recertifications, starts of care (SOC), resumptions of
care (ROC), other assessments and services, occasional Skilled
Nursing visits, as well as new hire orientations, trainings,
in-services, and supervisory reviews. At least 1 year of Case
Management experience and home health agency (or equivalent)
experience are required . Must have experience with OASIS and
NON-OASIS assessments. 1 or more years of experience in home health
care, wound care, lab draws, and OASIS assessments is a plus.
SERVICE AREA: Norwalk, OH 1-hour radius including Huron County and
Lorain County Part-Time: Typical travel distances require up to a
60-minute daily radius (or less). Full-Time: Additional cases can
be secured to achieve 32-40 hours/wk if willing to expand travel up
to a 2-hour radius to cover additional, surrounding areas and
counties, if desired (not required). JOB LOCATION OPTIONS: Norwalk,
OH in Huron County is centrally located to serve all cases within
this service area, however qualified applicants residing in or
adjacent to any of the defined counties will be considered so long
as travel requirements are fulfilled. EMPLOYMENT TYPE: Part-time or
PRN Hours | Long-term employment* No On-Call requirements / No
Overnight shifts required. Part-Time RNCMs receive hourly
compensation (paid weekly, on Fridays). If PRN Nurse: with hours
and shifts varying week to week, including some weeks having low or
no hours at all. HCN will expand this role into Full-Time (choice
of hourly or salary) for the right candidate willing to travel up
to a 2-hour radius. Weekend, Overtime, Respite & PRN services are
offered when available but are not required. *HCN will consider
short-term or temporary employment for those interested in
Part-Time / PRN / On-Call / Back-up. Please specify on your

Typical hours are between Monday - Friday (weekdays) during the day

self-motivated home health RN Case Manager (RNCM) / Nurse
Supervisor to join our agency and serve existing and returning
clients throughout Huron and Lorain Counties, and surrounding areas
in NorthEast Ohio. This position requires travel to multiple
counties but offers a consistent schedule to fit your personal /
family commitments (no overnight shifts or overnight travel). This
is a Part-time, long-term employment opportunity that can be built
to fit your schedule (with potential to expand into permanent
Full-time position after 30-90 days). PRN is also available. For

Part-time Hourly Wage: $35 - $40/hour Job Summary As an RN Case
Manager / Nurse Supervisor, you will be instrumental in
coordinating and providing patient care and ensuring optimal
outcomes. With core skills in managed care, patient assessment,
client care and treatment plan documentation, and discharge
planning, you will oversee patient services and provide expert
nursing care. Your premium skills in critical care and infectious
disease care will be valuable in handling complex cases, while your
relevant skills in home and community care and supervising
experience will enhance your ability to lead and support a
multidisciplinary team. You will ensure the successful
communication with and between our physicians, our agency staff,
service administrators, other caregivers and clinicians, and the
client his/her family. Reports to: Regional or Clinical
Administrator / Director of Nursing Responsibilities/Requirements
of an RN Case Manager / Clinical Nurse Supervisor include: RN
performs client assessments while directing, planning, and
initiating appropriate actions in home care situations. RN
manages/supervises a team of RN, LPN, Home Health Aide (HHA), and
Personal Care Aides. RN to Document/Communicate/Coordinate patient
status/change in condition to care team. RN to perform job duties
while promoting personal safety and a safe environment for clients
and coworkers. Willing to travel surrounding counties. Minimum of
one (1) years of nursing supervisory experience. Home care
experience preferred. Qualifications Graduate of an accredited
School of Nursing. Minimum of 1 year experience as a Registered
Nurse, with an active RN License / ability to practice in the State
of Ohio. CPR and First Aide Certifications required (or can be
obtained prior to hire date through our agency or accredited third
parties). 1 year Experience in case management and home health
required, with 2 years of home health experience highly preferred.
Experience in a supervisory role, wound care, lab draws, and OASIS
assessments is a plus. Willingness to travel throughout all stated
counties in this service area to care for clients is required.
Strong background in nursing, patient care, and confidentiality.
Strong attendance and dependability track record with previous
employers. Desirable character traits: reliable, honest beyond
reproach, professional, compassionate, detail-oriented (in care
documentation), accountable, friendly and accommodating,
respectful, and positive. Familiarity with HIPAA regulations and
medical terminology. Current Driver's License and reliable
