Licensed Clinical Social Worker (LCSW) Location: Grafton, OH Agency: Cynet Health Pay: $1,220 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Job Title: Behavioral Healthcare Provider 1 Profession: Behavioral Healthcare Provider Specialty: Staff Augmentation Duration: Approximately 6 months Shift: Day Hours per Shift: 8:00 AM - 4:00 PM Experience: Minimum years of experience required License: LSW/LPCC Certifications: Required certifications in the relevant field Must-Have: Must hold LSW/LPCC Description: Work with an interdisciplinary mental health treatment team. Complete screenings which include detailed mental health screens. Conduct individual and group clinical interventions as prescribed on the individual mental health treatment plan using clinical intervention strategies within their scope of practice and directed by supervision. Provide psychosocial group interventions as prescribed on the mental health treatment plan. Provide mental health liaison services to an assigned caseload, including monitoring the progress of treatment plan goals. Complete psychosocial needs and treatment assessments as part of interdisciplinary treatment planning. Provide and seek information relative to patient care and treatment. Make appropriate community referrals, per the mental health treatment team. Write reports and/or documentation to include electronic documentation of all contacts, progress note entries, discharge plans, and miscellaneous forms and reports as needed, per policy and protocol.
